A fellow presents research at the Annual College of Gastroenterology Conference
A year of dedicated research time under the one-on-one supervision of a talented mentor is transformative in students’ careers. The goal of the DDRF Fellowship program is to teach the exceptional student to be a physician and a scientist, rather than one or the other. Historically, physician-scientists have produced many of the major advances in combating disease. They unite the urgency of the ill patient with the power of the scientific method. These talented young people represent tomorrow’s leaders in medicine.

Research Focus: Cecilia is investigating MASLD and its implications in pregnancy. She is focusing on steatotic liver disease in patients both with and without PCOS, as well as obesity, and looking at pregnancy outcomes. She is also incorporating lipidomics and metabolomics, as well as nutritional assessments, into her research. In addition, she is focused on the epidemiology of delta hepatitis and HBV in New York City-based cohorts.

Research Focus: Dr. Savin-Shalom’s research is generally focused on the impact of nutrition in patients with IBD. Her main projects include evaluating the impact of malnutrition (both protein calorie malnutrition as well as overweight and obesity) on IBD related outcomes and the novel use of ultrasound to evaluate nutritional status in patients with IBD. Finally, she is researching safety and efficacy of GLP1 agonist medications in patients with IBD.

Research Focus: Tsion’s research is aimed at understanding and improving colorectal cancer screening efforts. She is currently working on the following projects: 1) studying the implementation and effects of direct access screening through the Mount Sinai CHOICE program, 2) analyzing the time from positive stool test to colonoscopy and its associated pathologies, and 3) understanding acculturation as it pertains to African-born immigrants and perception of colorectal cancer risk and screening adherence.

Research Focus: My research efforts are focused on validating intestinal ultrasound scoring systems for Crohn’s disease and ulcerative colitis in both children and adults, aiming to detect postoperative Crohn’s recurrence in adults and monitor children undergoing biologic therapy. Additionally, I seek to gain procedural experience in intestinal ultrasound to train other gastroenterologists and promote its use in IBD centers nationwide for disease monitoring.

Research Focus: Our lab is developing a model for the early detection of pancreatic ductal adenocarcinoma through the application of deep learning methods combined with expert domain knowledge to Electronic Health Records (EHR) data. Michael will be leveraging these techniques in a parallel effort to develop models for the early detection of gastric and esophageal cancer by converting EHR data into a machine-readable format followed by building and optimizing candidate models using modern machine learning methods. In the long term this project also has the potential identify novel predictors of gastric and esophageal cancer.

Research Focus: While CRC screening rates in NYC have improved greatly in the past two decades, achieving the National Colorectal Cancer Roundtable's goal of "80% in Every Community" will require further innovation in care delivery. The Mount Sinai Health System is leveraging its interconnectedness and high resource availability to develop a comprehensive screening program that incorporates patient navigation, digital navigation, EMR optimization, and systemwide practice changes. Parth analyzes data to determine screening program strengths and weaknesses, participates in ongoing program optimization, and hopes to develop community outreach strategies to engage historically under-screened populations.

Bariatric surgery, with its pronounced, consistent treatment effect allows for a unique opportunity for collaboration where many disease processes may be studied such as the relationship between the aging process and weight loss using the rate of telomere degeneration as a marker of aging.

Research Focus: My research focuses on precision medicine, a medical model that uses individual patients’ data, including their own genetics, to help determine the best approach to prevent or treat disease. Dermatologic manifestations, including atopic dermatitis and psoriasis, are known to be an adverse reaction induced by some IBD-related therapies. Specifically, we are interested in analyzing the skin profiles of biologic-naïve IBD patients before and during treatment with anti-TNF biologic therapy. The identification of distinct skin profiles has the potential to direct providers to prescribe more personalized therapeutic choices for the IBD patient population.

Research Focus: During the height of the pandemic, research focused on prevention and acute treatment of COVID-19 infection in patients with IBD and other immune-mediated inflammatory diseases, many of whom are on immunosuppressant medications. Now, as the Post-Acute Sequelae of SARS-CoV-2 (PASC) are increasingly becoming recognized and better defined, new questions about immune-mediated diseases and the impact of immunosuppressant medications require evaluation. In her research, Emily will study PASC outcomes in patients with IBD and the effects of immunosuppressant medications on those outcomes.

Research Focus: Ari was working to establish an online registry of patients at high-risk of developing pancreatic cancer, such as those with a significant family history of pancreatic cancer or with a personal or family history of a genetic syndrome known to be associated with pancreatic cancer. Through this online survey, physician-scientists hope to establish a better understanding of how family history, environmental exposures and inflammatory lesions in the pancreas contribute to the development of pancreatic cancer.

Research Focus: A growing body of literature suggests that alteration of the gastrointestinal microbiome may influence tumor development, response to therapy, and survival in patients with pancreatic cancer. Using the SEER-Medicare population-based cancer registry linked with Medicare Part D we will seek to understand whether antibiotic use impacts survival among patients diagnosed with pancreatic ductal adenocarcinoma (PDAC). We hypothesize that antibiotic use increases the risk of PDAC development, while antibiotic use following diagnosis improves survival. This research has the potential to improve the management of patients at increased risk of developing PDAC as well as those with a confirmed PDAC diagnosis.

Research Focus: We are delighted to welcome Minami back for a second year as a DDRF fellow. Minami has recently submitted a publication with data supporting the safety and continued clinically indicated use of common inflammatory bowel disease (IBD) medications during the pandemic and their potential role in the treatment of COVID-19 based on common transcriptional similarities between the inflamed intestines of IBD patients and severe COVID-19. In the coming year, she plans to focus on GI involvement of COVID-19 and mucosal immune changes both in the acute setting and following infection resolution.

Research Focus: While the incidence of colorectal cancer has been falling in adults aged 50 and older, there has been an increase incidence in adults under the age of 50. Some proposed factors responsible for the increasing incidence include BMI, lower levels of physical activity, antibiotic use, and environmental factors such as pesticides. Lewis is researching the feasibility of identifying the best way to enroll patients in a case-control trial to investigate the factors associated with the rising incidence of early-onset colorectal cancer (EO-CRC) in order to work towards controlling the risk at the population level.

Additionally, she analyzed the effects of pre- and post-operative C. difficile infection in patients with Ulcerative Colitis who have undergone Ileal Pouch-Anal Anastomosis.

Research Focus: Lauren's research is focused on understanding the epidemiology of hepatitis delta virus (HDV) infection and gaps in care in a New York City-based cohort. She is working on the following projects: 1) utilizing an existing New York City database to determine rates of HDV testing and positivity, evaluate the prevalence of clinical comorbidities and liver-related disease progression among patients with HDV, and assess the association between HDV positivity and social determinants of health, 2) conducting a retrospective study of patients with hepatitis B virus (HBV) and HDV infections in the Mount Sinai Health System, and 3) studying how the implementation of HDV reflex testing impacts rates of HDV screening among patients with HBV.

Research Focus: Regardless of whether surgery is recommended, the majority of pancreatic ductal carcinoma (PDAC) patients undergo standard chemotherapy, which unfortunately offers limited benefits, typically resulting in a median overall survival of less than 12 months. Recent research highlights the significant impact of the PDAC microbiome on disease progression and suggests that the tumor microbiota might affect the effectiveness of chemotherapy. My objective is to expand on existing research by exploring the potential effects of specific medications, like antibiotics and antifungals, on survival rates for PDAC patients undergoing standard treatment.

Research Focus: Jeanette is investigating fatty liver during pregnancy. Her project focuses on recruiting pregnant patients to assess the prevalence of fatty liver, as well as associated pregnancy outcomes. Through this research, we hope to gain a better understanding of the pathophysiology of fatty liver within the pregnancy setting.

Research Focus: Nicholas is investigating the effects of neoadjuvant immunotherapy on cancer cells, stromal cells, and infiltrating immune cells in colorectal cancer and hepatocellular carcinoma. His project focuses primarily on "window-of-opportunity" trials, during which patients receive neoadjuvant therapy prior to surgical resection. These studies provide a unique opportunity to study the effects of novel therapies on the tumor microenvironment and to identify factors that may allow clinicians to predict response.
